3. DEATH OF MEMBER'S FAMILY
   Sister (step), brother (step), grandparents, in-laws
   1. card
4. MEMBER HOSPITALIZED OR LENGTHY ILLNESS
   1. $25.00 for flowers
5. BIRTH OF MEMBER'S CHILD
